DIAA Sports Physicals
All student-athletes who plan on playing a sport for Saint Mark's for the 2025-2026 school year must have an approved DIAA sports physical form turned into the Main Office, Athletic Training Office, or Nurse's Office before the start of the season. You are not allowed to tryout, practice, or play in a game without a sports physical. You may get this physical exam through your own doctor or a Med Express (you only need one physical for the entire school year). The link for the blank forms may be found to the left.
