DIAA Sports Physicals and Rank One Information


Beginning this fall (Fall 2026), DIAA will begin using a new student-athlete management system, Rank One. This transition is designed to improve the overall experience for parents, coaches, and schools while ensuring efficient compliance with DIAA eligibility requirements and regulations.  If your child intends to play a fall sport at Saint Mark's, please follow the instructions below.  ALL FORMS MUST BE COMPLETED BY AUGUST 10, 2026.
 
Step 1: Get your athlete's physical.  Every student athlete must have a new 2026-27 DIAA Physical Form on file to play sports.  The physical form can be found here.  This will be the only form accepted for the 2026-27 school year. These forms must be dated after April 1, 2026, for the 2026-27 school year.
 
Step 2:  Create a RankOne account for your family.  To access RankOne, please use this link: https://saintmarkshighschool.rankone.com/Pro/Login.aspx.   Once logged in, you will be required to enter your Student's First Name, Last Name, and Date of Birth to claim your student(s).  You can claim multiple students within one account. Once you set up your account, you will not have to set it up again. The required DIAA eligibility forms can now be electronically signed by parents through this portal.
 
Step 3: Complete the forms.  Once you are in RankOne, navigate to the Dashboard.  From the Dashboard, on the menu on the left side of the screen, navigate to "Forms."  Click the blue hyperlink to be taken to each form to complete.
 
You need to complete the following forms inside of Rank One:
1)  DIAA Transfer Eligibility - This form asks for last year's school attendance.  Even if you're transferring or considering transferring, you must fill out this form.
2)  DIAA Acknowledgement Form - Parents have to list your child's sports and acknowledge the DIAA Concussion and Sudden Cardiac Arrest protocols.
3)  Pre-Participation Consent Form - This consolidated form includes a request for consent for your child's participation in sports and other waivers and consents to DIAA policies.
4)  Emergency Contact Card- This form requests contact information and medical insurance information in case of emergency.
5) DIAA Student Athlete Medical Card:  This is where you must upload page 5 of the physical form (the Student Athlete Medical Card) after the physical is complete.  Page 1 is instructions, page 2 is the pre-participation consent form submitted separately, pages 3 and 4 remain on file with your child's doctor)
 
Double-check that each form is submitted and shows as "Complete". (Green)

Stadium Rules for spectators at game events
  • No outside food or beverages

  • No backpacks or other bags are permitted. Purses are fine.

  • No animals allowed on campus except Saint Mark’s dog, Benny

  • No smoking on campus

  • No alcoholic beverages

  • Shirts are required to be worn at all times

Expectations for Spectators

Saint Mark’s High School is committed to creating an environment that is fan

friendly, safe for all people, and promotes sportsmanship for all teams. It is

important that every spectator be an exemplary role model and maintain the

highest standard of conduct at all times. Spectators represent their community,

school and team. The spectator must recognize that the purpose of educational

based athletics is to promote physical, mental, moral, social, and emotional well-being of all participants. The following behavior could result in ejection from the

stadium or arrest by law enforcement:

  • Any disruption to the game by fan actions

  • Obscene gestures

  • Abusive or foul language

  • Throwing objects of any kind

  • Intoxication or other signs of impairment from substance use

  • Fighting or threatening remarks to anyone

  • Shirts are to be worn at all athletic events both indoors and outdoors

  • Failing to follow the instruction of stadium personnel or police

 

Please note: Unauthorized tailgating is not permitted on Saint Mark’s campus.

 

Fans are encouraged to report any issues or concerns to game staff or state police.
