PHYSICALS
DIAA updated the pre-participation physical exam (PPE) form in March 2022. All student-athletes must complete four DIAA Forms after 4/1/22 in order to be eligible to particpate/compete (practice or try-out) for the 2022 - 2023 athletic year.
There are four forms that must be signed by the athlete, the parent/guardian, and/or doctor.
- DIAA Consent Form (athlete and parent/guardian only)
- Health History Form (athlete and parent/guardian only)
- Physical Form (requires doctor's signature after 4/1/22)
- Student Medical Card Form (athlete, parent/guardian, and doctor)
These forms must be completed after April 1st each year based on a physical performed by the signing physician within one year of the date of signature.
